2.2. Business Data

This page summarizes the key business data artifacts and information structures involved in Ontario Health eForms, as represented using HL7 FHIR® SDC.

2.2.1. Primary artifacts

  • Questionnaire (Form Definition)
    Represents the structure, questions, constraints, conditional logic, and rendering instructions of an eForm. Ontario Health supplies Questionnaires that conform to this IG.

  • QuestionnaireResponse (Completed Form)
    Represents a user’s answers to a Questionnaire. QuestionnaireResponses are produced by form-filling applications and support downstream processing and consumption.

2.2.2. Supporting artifacts

  • Terminology artifacts (ValueSets and CodeSystems)
    Provide controlled codes used for consistent rendering and processing (e.g., item controls, item types, styles, launch context codes, MIME types, topic codes).

  • Template and rich content assets (optional)
    Some Questionnaires may include or reference:

    • Library resources containing Liquid templates used to generate narrative summaries, and/or
    • Binary resources used to support rendering (e.g., images referenced by XHTML or interactive diagrams).

2.2.3. Data characteristics and expectations

  • Standardization and interoperability
    Forms are represented using consistent FHIR structures to support reuse across systems and vendors.

  • Traceability
    Questionnaires are versioned and QuestionnaireResponses reference the Questionnaire used for completion, supporting reliable interpretation of captured data over time.

  • Structured capture and downstream use
    Completed forms can be consumed:

    • as structured response content (QuestionnaireResponse), and/or
    • via derived resources created when extraction is performed, and/or
    • via a narrative summary generated using a Liquid template.
