This operation creates the patient record and his her assigned care programme

This resource defines the patient involved in the remote clinical monitoring. Details like patient's name contacts gender birth date country code and preferred languages will be used. The institution (i.e AMK Polyclinic) that manages the remote clinical monitoring for the patient and the group organization (i.e National Healthcare Group Polyclinics) that owns the patient record will also be stored.

This resource defines the vital signs readings of the patient. Details like the type of vital signs (i.e blood pressure) the readings for each vital sign (i.e systolic blood pressure diastolic blood pressure) the device guid that took the readings and the subject patient id. In the event the readings are missed the effective period will be used to store the start and end date of the missed readings with the data absent reason as unknown .

This resource defines the communication alert for the remote clinical monitoring of the patient. Details like the type of alert and the patient id will be used. The observation id will also be stored depending on the alert code (i.e Threshold and Missed Read ).

This bundle is used to hold 2 entries Patient and CarePlan resources for the creation of the patient and his her assigned clinical programme in a single transaction

This resource defines the clinical programme assigned to the patient. Details like the programme category the period of enrollment and the patient id will be stored.

This bundle is used to hold 1 or 2 entries Communication and Observation (optional) resources for the sending of communication alerts together with the vital signs readings (if applicable) in a single transaction

This operation serves three functions - submit communication alert together with vital readings for alert-code Threshold . - submit communication alert together with start end dates for alert-code Missed Reading . - submit communication alert only for other alert codes (for example Contact Patient etc).
